Step 1.
Download, edit (or manully write in information) and print one of the two documents above.
 
   
 
 
Step 2.
Photocopy or scan your AMA card if you already have one, or specify that you have just applied for one on the application.
  
   
 
 
Step 3.
Write a check for the full or pro-rated dues to R. C. Wingers, Inc.
  
   
Current Standard Membership Rate = $50.00
Current Honorary Rate = $37.50
  
 
Step 4.
Put the above in an envelope and mail to:
